Knowledge of bone conduction allows us to develop new, modern hearing aids. The best are bone conduction implants. There is an efficient sound transmission, improvement in the quality of life of patients, and implants are now available for children from 5 years of age. In addition to technical improvements to audio processors, there has been a significant reduction of complications.

Cochlear implants have gradually become a common and routinely indicated procedure since their inception in the Czech Republic in the 1990s. The gradual expansion of indication options has progressed to the current state, which we summarise in this paper. We describe the main indication criteria for prelingual and postlingual hearing loss and procedures for the examination of candidates for cochlear implantation.

In the Czech literature there is a lack of information about speech and language assessment in adult cochlear implant (CI) candidates and users. The article presents the issue of adult cochlear implantation from a speech and language therapist perspective. Examination of the hearing of the youngest children is completely different from the examination of older children and adults. A toddler does not cooperate, has no auditory experience and cannot communicate. Vestibular schwannoma is a posterior cranial fossa tumour, which is usually manifested by unilateral perceptual hearing loss, tinnitus, vertigo, facial tingling, etc. Therapy of vestibular schwannoma is chosen according to the size of tumour, its location and growth rate. Therapeutic procedures include microsurgical treatment and/or stereotactic radiosurgery and radiotherapy. Hearing loss is one of the most common somatic-functional impairments (Horáková, 2012). Because hearing perception is key to the development of the spoken language, children with hearing impairments are in the care of clinicians - Speech-Language Pathologists or Special Educators - Speech Therapists/Deaf Educators. This article aims to offer insight into the specifics of the job of a Speech Therapist/Deaf Educator with children with severe hearing impairments. From this group, we then separate the category of children operated on with cochlear implants. This is dealt with in a specific chapter.
Inner ear malformation

Inner ear malformation comprises a broad group of disorders, the main symptom of which is hearing loss. The disorder occurs during the embryonic development of the auditory system. Only about 20 % of malformations can be diagnosed radiologically. Hearing impairment is sensorineural and can be of a mild degree, e.g. in the case of an enlarged vestibular aqueduct. However, more often, severe hearing loss is found. Hearing correction depends on the severity of the child's hearing impairment. Cochlear implantation in children with congenital profound hearing loss is optimal at the age of 12 to 24 months.

Lipreading is one of the communication systems of people with hearing impairment. We encounter it when communicating with individuals with hearing impairments from an early age to old age, regardless of the type of hearing impairment or compensatory aid that the individual uses. The difference is in the degree of usage and understanding that results from the individual needs and capabilities of each person. Lipreading is a very demanding mental activity. Not every individual with a hea-ring impairment can lip-read or is able to learn to lip-read. However, its successful acquisition can play a major role in speech comprehension, education and everyday life.

What does a child actually hear? At what intensities and at what frequencies? The answers to these questions are extremely difficult, because a child is not able to describe his/her auditory perception to us, as an adult would. Of the many available methods, VRA - Visual Reinforcement Audiometry - has proven to be the most viable method in recent years. This method is able to complement information about auditory perception with the child's subjective reaction to sound.

Visual Reinforcement Audiometry (hereinafter referred to as "VRA") is one of the behavioural methods of hearing examination. The case studies aim to provide information on the importance of the aforementioned test and are focused on patients on whom VRA tests are most often performed. The results of the VRA test are beneficial, both in cases of a discrepancy between the results of objective hearing measurements (evoked potentials test, test for the presence of evoked otoacoustic emissions) and the statements by the child's parents, as well as in determining the hearing threshold curve with a hearing aid.
